A hospital credentialing office asks for current licenses and a certificate of insurance. Operations forwards the ZIP from the last request because assembling it took an afternoon. One license has since renewed. The malpractice period rolled. The packet is a photograph of March.

This is not learning the policy from the broker. That failure is that the live declarations page never sat at the clinic. This failure is that the clinic had the files and still shipped a frozen copy. Payers delay. Privileges stall. Billing waits on a date you already held.

A packet should be generated, not reconstructed

• Professional licenses in force for the named clinician
• DEA if they prescribe at the site
• Malpractice period on the current declarations page
• Facility credentials if the desk asked for the site, not only the person

If you have to rebuild the packet, you do not have coverage on file. You have an archive with a cover sheet.

Keep the live row. Print from it. Then send.

When a board issues a new certificate, replace the same row before anyone asks. The next request should be a print of what is current, plus the files sitting on those rows. Do not keep a parallel credentialing folder that drifts the week after you zip it.
