DEA registration renewal checklist for clinics
DEA renewal is a practitioner process. The clinic still needs a current certificate on file and enough warning to finish the renewal before prescribing stops. This checklist is for the facility side.
Know whose registration you are holding
List every practitioner who prescribes controlled substances at the site. Each has a registration. Do not treat a facility binder as if it were one shared DEA number.
- Name as printed on the certificate
- DEA number and registered address on the paper (kept on the file, not as separate calendar fields)
- Expiration date on the current Form 223 or successor certificate
- Who at the clinic prompts the practitioner to renew
Confirm the paper you actually have
Open the current certificate. Match the name, number, and date. If the file is a photo of a card, check both sides. Put that confirmed date on the facility calendar. Do not copy a date from last year’s spreadsheet.
Start from the clinic’s lead time, not the week it expires
Practitioners renew with DEA. Clinics get surprised when the reminder lived on a personal email. Set facility reminders early enough that a delayed login or an address change does not become a lapse. When the new certificate arrives, file it against the same person and retire the previous scan.
